What do you write in a first birthday card?

Address the parents as much as the baby: congratulate them on the year, say one thing you have watched the baby learn, and add a blessing for the child. The baby will not read it; the parents will keep it in the album, so write it for them.

Is it fine to wish only the parents on a first birthday?

Yes, and it is often the kinder choice. A line like "You two did it, a whole year" means more to a tired parent than a wish the baby cannot understand. If the card is from an elder, add a blessing for the child at the end.
